ឃ- សំបុត្រអាពាហ៍ពិពាហ៍របស់ឪពុក-ម្តាយ (ករណីទារក)
ង- លិខិតឆ្លងដែនថតចម្លង១ច្បាប់ (ករណីអ្នកបាត់ឬខូចលិខិតឆ្លងដែន)

ច- របាយការណ៍ពីប៉ូលីសបញ្ជាក់ពីការបាត់លិខិតឆ្លងដែន (ករណីអ្នកបាត់លិខិតឆ្លងដែន) ។
 

- សាមីខ្លួន ឬឪពុក-ម្តាយ ត្រូវដាក់ពាក្យសុំដោយខ្លួនឯង​។

- លិខិតធ្វើដំណើរបណ្តោះអាសន្ន មានសុពលភាព យូរបំផុតត្រឹម៣ខែ គិតចាប់ពីថ្ងៃចុះហត្ថលេខា។

The Temporary Travel Documents may be issued only to Cambodian nationals who have: 
- an expired passport that cannot be extended; 
- their passports lost or damaged and they need to travel to Cambodia urgently; and
- just been born abroad. 

Requirements:
- Completely filled and signed Temporary Travel Document application form (one copy)
- One bio-page copy of the Cambodian passport (if available)
- One copy of the Birth Certificate (required for newborn baby)

- One copy of parents' Marriage Certificate (required for newborn baby)
- One copy of parents' Identification Document (required for newborn baby)
- 4x6 cm-size photos (2 pieces) 
- Police incident report (original copy) in case of a lost passport only
- Travel itinerary (Optional).

Notice:   
- Processing time: 2 - 3 weeks
- The travel document is valid for/less than 3 months from the date of issue and is for single use only.
